How to make a claim

Step 1

All potential claimants, employers or individuals should either contact our Customer Care Department or request a claim form.
The following basic information will be required:

  • Name and Date of Birth of claimant
  • Unum Policy Number

Step 2

The potential claimant, employer or individual should complete the relevant claim form. Depending on the product type, different information will be required:

Group Life

  • Details of the deceased
  • Original birth, marriage and death certificates


Critical Illness

  • Details of the claimant
  • A full medical history


Income Protection

  • Details of the claimant
  • Details of the claimant's occupation
  • A medical history from the treating General Practitioner
  • Permission to obtain medical information

As soon as all of this information is received, the claim can start to be processed.

Step 3

The claim is assessed by one of our experienced claims handlers. In the case of Critical Illness and Income Protection, we may need to receive further medical evidence or an impartial consultant may be required to prepare an independent assessment of the medical condition. Should this be the case, Unum will arrange to pay for this Consultation and any reasonable travel costs. Also, we may arrange for one of our Vocational Rehabilitation Consultants to visit the claimant to discuss their condition and potential rehabilitation.

Step 4

If the claim is accepted, payment will be made. Group Life payments will be made to the Trustees of the scheme, Critical Illness payments to the claimant and Income Protection to the policy beneficiary. In the case of Income Protection, the claim will be regularly reviewed to confirm that the claim remains valid.

When to make a claim

It is important that we receive your claim form as soon as is reasonably possible. Failure to do so may jeopardise the payment of the claim.
